It seems weird to me that a recipe calling itself "Healthy Meat Loaf" would start with ketchup and sugar as its first two ingredients.

I'm finding recipe after recipe that puts ketchup into meatloaf. Which seems very strange to me. I've never put ketchup in meat loaf in my life, and can't think why I'd want to - though conceivably I might have ketchup with it.

It just seems wrong.

My mother used to put chopped apples in meat loaf, and I tend to think of that as normal - it added just a little sweetness and just a little tartness and kept the dish moist. I'm not seeing that in the online recipes, either.

Well, the sugar is a surprise to me. If I'm making beef-based meatloat, ketchup is a usual ingredient. Other than that, no real surprises.

If I'm making meatloaf using ground lamb, it gets egg, breadcrumbs, mustard, cumin, and I forget which other spices - oh, yeah, rosemary at least.

Hmm...

My mom's recipe uses ketchup on top of the meatloaf, which I like. I used that as inspiration for my elkloaf, which has a ketchup-based sauce. I use organic ketchup. With my other meatloaves, I do different things. Haven't tried apple yet; I'll have to keep that in mind. If you want to try my recipes, some of them are posted in my LiveJournal under "recipes." I think the elkloaf is the only one with ketchup, though some have other sauces. The Greek one I made with lamb, it goes with tzatizki sauce.
