Excellent choices! While I do like the Jack/Ianto kiss in Cyberwoman it was very one-sided for me to really appreciate it. After that, their kiss in End of Days was very nice, but it lacked passion - it was more of a "relief that you're alive type kiss". The Jack/Ianto kiss in "Adam" was very sweet and it hurts my heart that they don't remember it. And yes, I also think the To The Last Man kiss should've been in this list - I probably would have picked this one instead of the Cyberwoman kiss.
Maybe it's my inner shipper, but I really wish there was a Ianto/Tosh kiss somewhere in the show!
